Does this ladder apply to "dog to human" aggression only?

I think it can apply to most situations, although it'd be more complex with another dog, as you'd need to observe the behaviour of both of them and judge from there.
Demps doesn't go beyond the growl, but very often settles at the stiff-legged staring. Plus flagging. He's extremely dominant, but has never bitten a dog (aside from self-defence, when he was attacked as a puppy).

Kuchar hasn't bitten another dog, but he's started to "answer back" and it happens so suddenly I don't see it coming. He is as keen as he was as a puppy to greet any dog and hopefully play with them, but recently he has sniffed noses with another dog and a split second later he and the other dog are roaring and snarling at each other. Not every dog by any means, but some.
